The Roots Home Visiting Program

Roots is a family-centered educational program serving pregnant women and families with infants and toddlers, including children with special needs. This Program is provided at no cost to families. We partner with your family to promote healthy child development and strong family connections.

We know being a parent is hard work. You help shape your child’s future through the kinds of experiences you provide. By understanding each stage of development, you can easily capture the teachable moments in everyday life to enhance your child’s language development, intellectual growth, social development and motor skills.

Family Mentors:

At Roots, we believe that you are and always will be your child’s first and most important teacher. Family Mentors partner with you to support your child’s development. They will plan and conduct quality, weekly home visits with you and provide you with resources, health and developmental information. Family Mentors are highly trained in the Growing Great Kids curriculum as well as current child development research. This curriculum provides Family Mentors with research-informed, strength-based and solution focused “Home Visit Conversation Guides”. These guides are designed to assist the Family Mentor in helping parents reflect on current parenting practices or experiences, share important information that will help parents support their child’s development and help build secure parent-child attachment.  Family Mentors will respect your family’s values, culture and privacy.

Home Visits:

Your Family Mentor will arrive at your home each week, at a time that is convenient for you (all visits can be rescheduled at your request). They will bring a book for you and your child to read together during the visit as well as an activity for you and your child to engage in. You and your Family Mentor will plan for your next visit together. Home visits are also good times for you and your Family Mentor to discuss family goals and meeting your changing needs. Each visit lasts an hour and a half.

Events:

We also offer Parent and Child Together Events (PACT), where families have the opportunity to share information about parenting challenges and child development. Parents learn and support each other, observe their children with other children and practice parenting skills. During events, Roots staff will provide fun, family strengthening and educational activities. Attending PACT events is also a great opportunity for you and your child to focus on school readiness goals and help strengthen your ties to the educational community.

Screening and Monitoring:

Roots offers screenings to assess your child’s overall health and development. We use the Ages and Stages Questionnaire (ASQ) and the Ages and Stages Questionnaire Social Emotional (ASQ-SE) to screen for possible developmental delays. Your Family Mentor will monitor your child’s growth and development using Teaching Strategies Gold and the Devereux Early Childhood Assessment for Infants and Toddlers (DECA). Family Mentors also provide vision, hearing and dental screens in your home. All screening and assessment results will be shared with you. We also provide nutrition screens and post-partum depression screens for expecting mothers.

Community Resources:

Roots offers an extensive resource network that links your family to community services including local and early childhood mental health clinicians. We partner with The Buena Vista Pregnancy Center, The Optimist Club, Chaffee County Early Childhood Council, and Chaffee County Public Health in order to help families find the services and information they need.
